Written reports

Each agency invited to attend the conference should produce a written report and discuss this with parents, and children where appropriate, in advance of the conference. It is the responsibility of all agencies, who have relevant information, to make this available to the conference in the form of a written, legible and signed report.

Timescales

It is important that families do not hear new information at the conference. Reports should be discussed with parents well before the conference and their views noted. Once the report has been discussed with the parents, a copy should be provided to the conference chair at least 48 hours before an initial conference and 5 working days before a review conference.

Reports should address the areas highlighted in the pdf icon My World Triangle [144kb] .Report writers should address the needs of each child within the family separately even though only one report is produced. They should check their records for factual data such as immunisation history, clinic attendance, school attendance etc.. Relevant adult records should also be scrutinised. Completed reports should be sent by secure email to: Helen.LeTocq@gov.gg, for the attention of the conference chair. There is a suggested report format available below.
